Portland, United States – Sept 11, 2026 – Synergy Women’s Health is urging Portland-area women to bring bone health into their perimenopause care before the window for meaningful action narrows.
Synergy Women’s Health, a gynecology and women’s health practice located in the Lovejoy Medical Building in NW Portland, is drawing attention to a pattern that surfaces regularly in midlife care: bone health tends to enter the picture only after something has already gone wrong.
Biology explains the timing problem. Estrogen actively regulates the balance between bone formation and bone resorption. As estrogen levels begin declining during perimenopause, that balance can shift for years without producing a symptom a woman would notice or think to mention at a routine visit. There’s no pain to catch it early. No gradual signal to track. The process runs quietly while everything else feels fine.
That’s not a reason for alarm. It’s a reason to have the conversation sooner than most care plans currently schedule it.
Why the Conversation Gets Deferred The concerns that bring women in during perimenopause are ones they can feel: disrupted sleep, irregular cycles, mood changes, hot flashes. Those deserve direct clinical attention. Bone metabolism doesn’t announce itself the same way, so it tends to fall outside what a standard visit covers unless a provider is already looking for it.
Consider a woman in her mid-forties who comes in primarily about sleep disruption. Her symptoms are worth addressing on their own terms. But her family history, current medications, or other aspects of her clinical picture might mean her bone health also calls for a closer look right now, not at a screening milestone that’s still years away. A visit organized only around what’s visibly bothering her today won’t surface that larger picture on its own. Waiting for the standard screening age can feel like the cautious choice. It isn’t always.
How Synergy Approaches It Perimenopause care at Synergy Women’s Health is built to hold both the immediate symptom picture and the longer view at the same time. Every evaluation starts with a thorough history, a physical exam, and appropriate laboratory work. The practice begins with conventional serum testing and moves toward additional evaluation when the clinical picture calls for it, working to make full use of insurance coverage at each step. No two treatment plans here look alike. That’s deliberate. Care is built through genuine collaboration with the patient, grounded in education and informed decision-making, rather than a protocol applied the same way regardless of who’s sitting across the table.
The Provider Team For patients whose concerns call for a deeper integrative evaluation, Dr. Michelle Sang leads that work directly. Dr. Sang is a board-certified OB/GYN and board-certified in Anti-Aging Medicine, holding both her FAARM and FACOG designations. She earned her BA from Harvard University and her MD from OHSU and has been practicing women’s health as an OB/GYN since 1994. Her functional medicine program is structured around an extended initial consultation, a comprehensive diagnostic workup, and individualized treatment plans that address nutrition, lifestyle, supplements, and medications when appropriate, with follow-up appointments scheduled every two to three months and adjusted as the clinical picture evolves. The program operates on a membership basis, with patients choosing between a quarterly or annual fee. All office visits, regardless of the reason, are billed to insurance, so patients make full use of their existing coverage.
Dr. Karen Ogryzlo, co-owner and board-certified OB/GYN, and Sangita Ghimire, who brings over seven years of women’s health experience and is fluent in English, Nepali, and Hindi, provide conventional gynecologic care across the practice. Both incorporate functional medicine perspectives into their work under Dr. Sang’s guidance and refer patients to her when a deeper integrative evaluation would better serve them. Care stays coordinated. Patients don’t have to start the story over somewhere else.
A Realistic Note A deeper integrative workup isn’t the right fit for every patient, and Synergy doesn’t treat it as though it is. Conventional gynecologic care at the practice addresses a broad range of needs without requiring that level of evaluation. The more thorough integrative approach is specifically for patients whose symptoms, history, or contributing factors genuinely call for it. What the practice doesn’t do is treat bone health as a topic to raise only after a fracture brings it to the surface. That conversation belongs in perimenopause care from the beginning.
